How Portuguese pharmacies work

Pharmacies (farmácias) in Portugal are tightly regulated and play a much larger role than US drugstores: pharmacists can advise on minor conditions, take blood pressure, do quick blood-glucose tests, give certain vaccines, and even sell some normally-prescription items in small quantities. Treat them as your first stop for anything minor — it'll save you a doctor visit.

Brand names are different

Active ingredients are universal; brand names rarely cross borders. Two ways to find your medication:

• Look up the international non-proprietary name (INN) on the Infarmed database (infarmed.pt) to find Portuguese brands
• Show the pharmacist the original packaging or a photo — they recognise most major US/UK brands
• Use the Medisys or Drugs.com cross-reference to translate brand → INN → Portuguese brand

What's over the counter

Behind the counter but no prescription needed (in small quantities, at the pharmacist's discretion):

• Topical antibiotic creams (mupirocin, fusidic acid)
• Ondansetron (for nausea)
• Most NSAIDs and stronger painkillers (ibuprofen 600 mg, diclofenac)
• Loperamide, omeprazole, antihistamines, basic anti-fungals
• Emergency contraception

Costs

Portuguese drug prices are among the lowest in Europe, even before any reimbursement.

• With a Portuguese prescription + Utente number: SNS reimburses 37–95% at the counter (chronic conditions get the highest rate)
• Without a prescription: full price, still typically 30–60% lower than US retail
• Ask for the 'preço de venda' on the box vs. what you pay — the difference is the reimbursement

Renewing chronic prescriptions

Prescriptions are now electronic — you receive a code by SMS or email and present it at any pharmacy. Most chronic medications can be renewed for 6–12 months at a time and refilled monthly without seeing a doctor again. Set a calendar reminder a week before the last refill so you don't run out over a weekend.

Out-of-hours pharmacy

Pharmacies rotate through a 'farmácia de serviço' schedule — at any moment one in your area is open 24/7. Find tonight's by:

Vaccines and travel

Pharmacies administer flu, COVID, pneumococcal, shingles and most travel vaccines without a doctor's appointment. Centros de Saúde do them too, often free for residents under SNS schedules.
