Why annual exams are so popular here — and how to take advantage.

An unusual cultural feature
Portuguese adults — especially over 40 — tend to get a comprehensive 'check-up' every year. Private hospitals package these as standardised bundles, advertised on TV and in metro stations. Coming from a US system where annual physicals are increasingly minimal, this is a pleasant surprise: you can get a thorough work-up for the price of a single US specialist visit.
What a typical check-up includes
Bundled by age and gender. A 'check-up básico' (€150–€250) usually covers:
- Comprehensive blood panel (CBC, lipids, glucose, liver, kidney, thyroid)
- Urinalysis
- ECG and resting blood pressure
- Abdominal ultrasound
- Chest X-ray (over 40)
- GP consultation to interpret results
Where to get one
Almost every private network sells them:
- CUF — 'Check-up CUF', the most established and rigorous, multiple tiers
- Lusíadas — 'Check-up Lusíadas', strong cardiac focus
- Hospital da Luz — premium-positioned, broadest packages
- Joaquim Chaves — value-priced packages, available across greater Lisbon
Why it's so affordable
Three reasons Portuguese check-ups cost a fraction of US equivalents:
- Lab and imaging costs are far lower (regulated pricing, lower labour costs)
- Hospitals compete on volume and cross-sell follow-up care from any findings
- The bundles are designed to fill quiet morning slots — efficient capacity use
How to use one well
A check-up is only useful if you act on the results.
- Pick the package level that matches your age and risk factors — 30 with no family history can do basic; 50+ should do mid-tier or premium
- Schedule it on a quiet morning; allow 3–4 hours
- Fast 8 hours before (water OK)
- Bring a list of meds and any prior abnormal results
- Get the PDF report afterwards — many specialists will accept it instead of repeating tests
- Share with your regular GP for interpretation in your full context
Insurance angle
Most Portuguese private health plans include one annual check-up at near-zero co-pay (sometimes free at certain tiers). Read your apólice — you may already be paying for one and not using it.
